Responda is assistive documentation software for EMS clinicians. These terms describe what it does, what it explicitly does not do, and who is responsible for what.
1. What Responda is
Responda is a workflow tool for producing electronic patient care reports. It transcribes recorded audio from a call and proposes structured values a clinician may accept into a chart.
Every proposed value is a suggestion until a clinician accepts it. Nothing the software extracts enters a patient record on its own. The clinician who accepts a value is the author of that value.
2. Responda is not a medical device and not clinical judgment
Responda does not diagnose, treat, or recommend treatment. It does not alert, triage, or prioritize patients. It produces documentation of care that a clinician already delivered and already decided upon.
Extraction accuracy is validated against synthetic scenarios. It has not been validated against production clinical data, and no accuracy figure we publish should be read as a guarantee for any individual chart. A clinician must read every value before accepting it.
3. The agency remains the custodian of the record
The patient care report is the agency's legal record. Responda stores and processes it on the agency's behalf. The agency is responsible for the accuracy and completeness of its charts, for meeting its own state reporting obligations, and for its retention schedule.
Responda does not file reports with any state registry on an agency's behalf unless a separate written agreement says so.
4. Protected health information
Charts contain protected health information. Where Responda processes PHI on an agency's behalf, it does so as a business associate under a separate Business Associate Agreement. No agency should place real patient information into Responda before that agreement is signed by both parties.

5. Accounts and access
Accounts are issued to named individuals. Credentials must not be shared. An agency administrator is responsible for removing access when a member of their workforce leaves or changes role, and for reviewing who holds access.
You must tell us promptly at hello@responda.us if you believe an account has been compromised or a chart has been accessed by someone who should not have seen it.
6. Acceptable use
You agree not to:
- enter information about a patient you are not authorized to document;
- use the software to store records unrelated to emergency medical care;
- attempt to access another agency's data;
- reverse engineer, resell, or provide the software to a third party without written agreement;
- use extracted output as a substitute for reading the chart before signing it.
7. Availability
Responda is provided without a uptime commitment unless an agency's written agreement states one. Documentation is time-sensitive and networks fail; an agency must retain a workable fallback for producing a patient care report when the software is unavailable.
8. Service changes and termination
We may change the software. Where a change materially reduces functionality an agency relies on, we will give notice before it takes effect.
On termination, an agency may export its charts. We will return or delete agency data according to the Business Associate Agreement and applicable retention obligations. Ask us for an export before terminating rather than after.
9. Warranties and liability
Except where a signed agreement states otherwise, the software is provided "as is" without warranties of any kind, including fitness for a particular purpose.
Nothing in these terms limits liability that cannot be limited by law, including liability arising from a party's own gross negligence or willful misconduct, or an agency's obligations under health privacy law.
